In Agile risks are identified continuously during sprint planning, demos, retrospectives, and daily stand-ups.
Involve stakeholders when identifying risks
Determine who has insight into risks. Involve stakeholders and customers. Project teams are great at identifying risks, if they are trained, given time for risk management activities, and if there is a no-blame culture.
Risk management is everyone's job, but it depends strongly on the managers how risk management activities are carried out.
